Liberty Cares With Compassion At Liberty Home Car e and Hospice we know that following an illness, trauma or surgery, the ability to recover at home can greatly improve patient outcomes. Our healthcare professionals are dedicated to offering recovery with independence to our patients. We are currently seeking an experienced:
REGIONAL MEDICAL SOCIAL WORKER PRN
(Ahoskie/Swan Quarter)
Job Description:
Provides patient care services in regional group of Liberty Home Health & Hospice offices and documents patient care data. Provides initial psycho-social assessment for the development of an individualized care plan and establishes follow-up treatment goals. Applies a working knowledge of psycho-social principles and family dynamic development. Identifies and reassesses social, emotional, financial, and environmental factors which may affect the medical plan of treatment and desired outcome. Communicates as necessary with intercompany and intra-company contacts. Utilizes community resource planning and community organization skills in providing patient services. Educates patients/families on how to utilize community resources available and provides discharge instructions. Other job duties as assigned.
Job Requirements:
Master's degree from a school of social work that is accredited by the Council on Social Work Education and eligible for certification by the North Carolina Board of Social Work. Baccalaureate degree in social work, psychology, sociology, or other field related to social work, and has at least one (1) year of social work experience in a healthcare setting Minimum of one year practical experience, preferably in the home health care environment. Strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting. Must accept travel and working conditions as assigned.
