The clinician will deliver individual therapy sessions, crisis intervention services, and psychosocial rehabilitation in home and community settings while conducting thorough biopsychosocial assessments and developing individualized treatment plans. Primary responsibilities include coordinating care with psychiatrists, nurses, and multidisciplinary team members to monitor behavioral health symptoms and support medication adherence.
