Job Description

Responsibilities:
- Assess psychosocial status and communicate findings Plan interventions, provide referrals, and serve as liaison Participate in care planning and discharge processes Support team with understanding social/emotional factors
Qualifications:
- Master's in social work from an accredited program At least one year healthcare experience Strong communication skills, reliable transportation, valid driver's license Recent physical exam, TB test, proof of flu vaccination We are committed to diversity and equal opportunity.
