The LCSW works collaboratively with physicians, nurse practitioners, psychologists, and other healthcare professionals to support patients experiencing behavioral health, emotional, social, and psychosocial challenges. This position focuses on improving patient outcomes through evidence-based interventions, patient advocacy, and coordination of community resources. Responsibilities

• Conduct comprehensive biopsychosocial assessments and mental health evaluations.
• Develop individualized treatment plans based on patient needs, goals, and clinical findings.
• Provide individual, family, and group therapy using evidence-based therapeutic modalities.
• Diagnose behavioral health conditions within the scope of licensure and applicable regulations.
• Perform crisis intervention and risk assessments, including evaluation of suicidal ideation, self-harm, abuse, and other behavioral health emergencies.
• Maintain accurate, timely, and compliant clinical documentation in the electronic health record (EHR).
• Coordinate care with primary care providers, psychiatrists, specialists, and community agencies.
• Educate patients and families regarding mental health conditions, coping strategies, treatment options, and community resources.
• Facilitate referrals to community support services, social service agencies, substance use treatment programs, and other healthcare resources.
• Advocate for patients to ensure access to appropriate services and continuity of care.
• Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
• Ensure compliance with HIPAA, state licensing requirements, clinic policies, and ethical standards of social work practice.
• Maintain current knowledge of best practices, regulatory requirements, and emerging behavioral health trends Education and Experience
• Master's degree in Social Work (MSW) from an accredited program.
• Minimum of two (2) years post-licensure clinical experience.
• Experience working with underserved, low-income, or culturally diverse populations preferred.
• Experience in an FQHC, community health center, integrated care setting, or primary care environment preferred. Licensure
• Must possess current and unrestricted Licensed Clinical Social Worker (LCSW) license in Nevada. Skills and Abilities
• Knowledge of DSM-5 diagnoses and evidence-based therapeutic interventions.
• Strong assessment, treatment planning, and crisis intervention skills.
• Ability to work effectively within a multidisciplinary healthcare team.
• Understanding of trauma-informed care principles.
• Knowledge of social determinants of health and their impact on patient outcomes.
• Excellent communication, documentation, and interpersonal skills.
• Ability to manage a diverse caseload in a fast-paced healthcare environment.
