Full-Time Posting Detail Information Position Summary UBMD Pediatrics, in the Department of Pediatrics in the Jacobs School of Medicine and Biomedical Sciences at the University at Buffalo (UB) is actively recruiting for a full-time compassionate Licensed Clinical Social Worker (LCSW) to support the Pediatric Pulmonology and Nephrology divisions. This role provides psychosocial assessment, care coordination, and resource navigation for children and families managing complex pulmonary and renal conditions. The social worker will collaborate with multidisciplinary teams to support patient wellbeing, treatment adherence, and access to community resources.Key Responsibilities include:
- Conduct psychosocial assessments and provide ongoing support to patients and families;
- Coordinate care and connect families with hospital, community, and public resources (transportation, insurance navigation, financial assistance, disability benefits, etc.);
- Participate in multidisciplinary care planning and case conferences;
- Provide education and support related to illness adjustment and treatment adherence;
- Serve as liaison with schools, community agencies, and Child Protective Services when needed;
- Assist families with documentation including FMLA, work accommodations, and transition planning to adult care;
- Maintain timely documentation and ensure compliance with institutional and regulatory standards;
- Participate in quality improvement initiatives, departmental projects, and professional development activities.
- Licensed Clinical Social Worker (LCSW) in New York State.
- Master's degree in Social Work (MSW). Preferred Qualifications
- Experience working with children, adolescents, or families in healthcare or community settings preferred.
