Job Description

Job Summary Axzons is seeking a dynamic and experienced Licensed Clinical Social Worker (LCSW) to support patient intake, care coordination, and referral development . This role is both clinical and growth-oriented , focusing on: Building referral relationships Supporting Medicaid / MLTC enrollment Ensuring high-quality patient care coordination The ideal candidate is proactive, relationship-driven, and experienced in New York home care or MLTC environments . Responsibilities 1. Patient Intake & Care Coordination Conduct comprehensive psychosocial assessments for new patients 2. Referral Development & Outreach (CRITICAL) Build and maintain relationships with: Hospitals & discharge planners Rehabilitation centers Nursing homes Physicians & clinics Community organizations Represent Axzons in the community to generate new patient referrals Conduct facility visits and outreach meetings regularly 3. Medicaid & MLTC Support Assist patients and families with: Medicaid application and eligibility guidance MLTC plan selection and enrollment Coordinate with MLTC plans such as: Centers Plan VNS Health Fidelis MetroPlus Ensure timely submission of required documentation 4. Case Management & Retention Provide ongoing emotional, social, and resource support to patients Resolve patient/family issues to ensure high satisfaction and retention Act as key liaison between: Patient Family Agency MLTC plan 5. Rapid Intake & Service Activation Ensure fast onboarding (within 24-48 hours) for new cases Collaborate with scheduling team to ensure caregiver placement Support urgent and high-priority cases 6. Compliance & Documentation Maintain accurate and compliant documentation Ensure adherence to: NY DOH regulations Agency policies MLTC requirements
Qualifications Required:
Active New York LCSW license Minimum 2+ years experience in: Home care / MLTC / hospital discharge Strong knowledge of: Medicaid MLTC plans in NY Excellent interpersonal & communication skills Ability to travel locally (field visits)
Preferred:
Existing relationships with hospitals or community providers Bilingual (Spanish, Russian, Hindi, etc.) Develop and implement individualized care plans Coordinate care with: Nurses (RN/DON) Caregivers MLTC care managers Monitor patient progress and adjust services as needed At Axzons this is NOT a back-office role.
