Medical Social Worker 9711 E Pike Rd, Cambridge, OH 43725 Full-time Full-time Overview We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team at Hospice of Guernsey. This role involves providing psychosocial support, counseling, and case management services to patients across various settings, including inpatient, outpatient, hospice, and community care. The ideal candidate will have experience in behavioral health, mental health counseling, and working with diverse populations and those facing end of life challenges. The Medical Social Worker will play a vital role in enhancing patient care by addressing social determinants of health, coordinating resources, and supporting patients and families. Responsibilities Conduct comprehensive patient assessments to identify psychosocial needs and develop individualized care plans Coordinate with healthcare teams to facilitate discharge planning, home & community care, and post-acute hospice care services Manage case documentation accurately within EMR systems while adhering to HIPAA regulations and medical documentation standards Collaborate with multidisciplinary teams to ensure comprehensive patient care including hospice care and assisted living facilities Maintain ongoing communication with patients regarding intake procedures and social work interventions Requirements Master's degree in Social Work (MSW preferred) from an accredited program; licensure or certification as required by state regulations Bachelors degree in Social Work required from an accredited program Proven experience in clinical social work within hospital settings, nursing homes, outpatient clinics, or home & community care environments Knowledge of medical terminology, medical records management Familiarity with HIPAA compliance standards and confidentiality protocols for medical documentation and records management Excellent communication skills for intake interviews and interdisciplinary collaboration Experience working with post-acute care settings such as hospice care, nursing homes or assisted living facilities is preferred This position offers an opportunity to make a meaningful impact on patient lives through compassionate support and expert clinical intervention.
