Job Details:
The BSW Care Coordination Medical Social Worker assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional, and physical health. Practice professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. These activities are performed in a variety of settings including during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, and community visits, as well as telephone and "virtual" visits. Supervising the Outreach Coordinators within the HRA program.
Education:
Bachelor's degree in Social Work required
Certifications:
LSW Ohio
- Ohio Licensed Social Worker
- Counselor, Social Worker and Marriage & Family Therapist Board required BLS required within the first 60 days of hire
Experience:
Children services and child welfare preferred Experience within the NICU preferred
Department Specific Job Details:
full time, 40 hours Monday
- Friday day shift, 8:00 AM
