MEDICAL SOCIAL WORKER A
Path of Care Hospice in Norman, OK is seeking a PRN Medical Social Worker (MSW, LCSW) to support patients and families with compassionate, skilled social work services. What You'll Do as a Medical Social Worker As a Medical Social Worker, you help patients and families navigate the emotional, social, and practical challenges that accompany serious illness.Your responsibilities include:
Assessing social and emotional factors affecting patient health Participating in the development of individualized plans of care Maintaining accurate clinical and progress notes Identifying and coordinating community resources to support patients and families Assisting with discharge planning Attending in‑service trainings as needed Consulting with interdisciplinary team members to ensure well‑rounded patient care Practicing infection control and following universal precautions Recognizing and reporting any signs of abuse in accordance with policy and law Assisting with office tasks and new‑staff orientation when needed Your work helps ensure patients receive holistic, compassionate care that supports both their medical and emotional needs. Qualifications Master's degree in social work from a CSWE‑accredited program At least 1 year of social work experience in a healthcare setting CPR certification or ability to obtain within 60 days of hire LCSW preferred, though multiple factors will be considered Strong verbal and written communication skills Excellent organization, time management, and problem‑solving abilities Ability to remain calm and professional in stressful situations Friendly, compassionate demeanor and ability to work well independently and as part of a team Work Schedule Full‑time Monday-Friday, 8:00 AM - 5:00 PM Ready to Join Our Team?
