Job Description

Job Summary Samaritan Health Services is seeking a compassionate and dedicated Hospice Outpatient Bereavement Coordinator to provide grief and bereavement support to families and loved ones served by Evergreen Hospice. This role is ideal for a behavioral health professional with experience in grief counseling who is passionate about supporting individuals through loss and healing. Evergreen Hospice serves patients across Linn, Benton, Marion, Polk, Lincoln, and Tillamook counties, providing comprehensive hospice care in patients' homes, skilled nursing facilities, assisted living communities, and adult foster homes. Responsibilities Provide individual and group bereavement support to families and caregivers. Conduct grief assessments and develop appropriate support plans. Coordinate bereavement services and community resources. Collaborate with interdisciplinary hospice team members. Maintain accurate documentation in accordance with agency policies. Participate in program development and community outreach as needed. Travel throughout the service area to provide support services. HL01
Pay:
$60.00 - $63.00 per hour Application Question(s): 1. How many years of experience do you have working in hospice, palliative care, or a healthcare setting? 2. Do you have a Bachelor's degree in Social Work, Counseling, Psychology, or a related field? 3. Do you currently hold a valid BLS certification 4.
