Location:
Northeast Oregon A premier hospital in beautiful Northeast Oregon is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join their expanding behavioral healthcare staff. Operating on a highly desirable schedule of four 10-hour days, this role offers an exceptional work-life balance in a supportive, collaborative environment. Key Responsibilities:
Conduct comprehensive psychosocial assessments, mental health screenings, and diagnostic evaluations for patients and families. Provide evidence-based crisis intervention, short-term counseling, and medical social work services to support patients dealing with acute or chronic medical conditions. Partner directly with physicians, mid-level providers, and nursing staff to coordinate seamless care transitions between inpatient units, outpatient clinics, and community-based resources. Support, counsel, and educate patients and their families on disease pathology, coping mechanisms, and complex healthcare decisions. Identify barriers to health and safety, facilitate emergency interventions when necessary, and arrange for post-discharge care, including home health, rehabilitation, or medical equipment. Maintain accurate, timely, and secure documentation of all patient encounters within the electronic health record system for billing and strict continuity of care. Participate actively in multidisciplinary team rounds to discuss complex cases, optimize clinical outcomes, and champion holistic patient wellness.
Qualifications:
Master of Social Work (MSW) degree from an accredited institution Current, active license as a Licensed Clinical Social Worker (LCSW) or eligible for immediate licensure in the state of Oregon 1-2 years of clinical social work experience, preferably within a hospital, clinic, or integrated healthcare setting Comfort with and a genuine interest in living and practicing within a rural community setting Excellent crisis intervention, communication, and interpersonal skills with a strong collaborative mindset
