  • Position Overview
  • The Clinical Licensed Social Worker (LCSW) provides high-quality behavioral health and therapeutic services to participants in the PACE program.
This role conducts comprehensive psychosocial assessments and delivers evidence-based interventions—including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and psychodynamic therapy—to support individuals experiencing trauma, anxiety, depression, and adjustment challenges. Working independently and as part of an interdisciplinary team (IDT), the LCSW develops, implements, and monitors individualized care plans. The role also provides crisis intervention, coordinates with physicians and care teams, and ensures accurate, confidential documentation in compliance with HIPAA and regulatory standards.
  • Key Responsibilities
  • Deliver individual and group counseling services
  • Participate in interdisciplinary care planning and coordination
  • Provide crisis intervention and participant advocacy
  • Educate participants and families on aging, grief, dementia, and care resources
  • Collaborate across teams to ensure person-centered care
  • Serve as a behavioral health resource for staff and team members
  • Maintain compliance with policies, regulations, and quality initiatives (QAPI)
  • Qualifications
  • Master's Degree in Social Work (MSW) required
  • Current Licensed Clinical Social Worker (LCSW) license
  • Two (2) years of experience preferred, including work with frail or elderly populations
  • Experience in multidisciplinary healthcare settings preferred
  • Knowledge of Medicare, Medicaid, and social service practices
  • Reliable transportation required
