Job Description
Medi Home Hospice , a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Medical Social Worker for our Hospice Patients in State College, Altoona, Clearfield (Centre, Blair, Clearfield), PA.
- Provides social work services in accordance with the physician's plan of treatment.
- Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
- Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
- Serves as a liaison with other community resources. Job Requirements
- Master's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
- Minimum of one-year Hospice experience preferred.
