Job Description:
Professional Clinical Social Worker Clinical/Behavioral Health Job Purpose:
Provide social services and assistance to improve the social and psychological functioning of adults, children, and their families. Social Workers in this field assess their client's needs and aid to improve their situation. This often includes coordinating available services to assist a child or family.
Duties:
Provide an assessment of the individual or family for service needs and linkage to available local community resources for the purpose of preventing or remedying or assisting in the solution of problems. Uses appropriate tools to make assessment and evaluations; prepares summary and makes recommendations regarding program services; communicates information to treatment team; maintains knowledge of updated program changes; maintains knowledge of DSM V patient placement criteria for appropriate placement and continuing care. Conducts individual, and family therapy, outreach, crisis intervention, to improve social rehabilitation. Teaches skills needed for everyday living and helps clients who face a disability, life threatening disease, or social problems, such as inadequate housing, unemployment, or substance abuse. Develops treatment plans with clients, encouraging them to create and pursue goals for improving their lives. • Provides services to assist family members of those who suffer from addiction or other mental health issues. Identifies potential conflicts, implements intervention techniques to prevent or intervene in potential crisis situations and assists families that have serious domestic conflicts. Uses clinically appropriate professional terminology to complete forms and records, accurately and timely documenting member assessment, planning, and progress; maintain knowledge of laws and regulations surrounding confidentiality of alcohol and drug patient electronic medical record and paper charts. Comfortable working within the scope of telephonic care services (video) and be famililiar with woking on Electronic Medical Record (EMR) platform. Develops and maintains appropriate professional boundaries between self, staff, members, and collaterals and identifies issues related to ethics and boundaries, as identified in the Code of Ethics policy and adheres to confidentiality requirements. Maintains appropriate credentials and pursues internal and/or external training and development opportunities. Performs related duties as assigned and attends mandatory in-services. • May be required to travel to schools, hospitals, or other treatment care facilities. Skills/Qualifications Must possess excellent communication, organizational, independent, and interpersonal skills. Personal characteristics of maturity and professionalism with a sincere desire to work with recovering individuals in a social service environment. Should be comfortable using a computer, its software, and all other office equipment. Basic computer skills in word processing, e-mail, and Internet use. Must be proficient in Microsoft computer programs.
Required Education:
Masters Degree, or higher degree in Social Work at an accredited University or College. Knowledge of social services, mental health, and co-occurring disorders characteristics, assessment, and care planning. Higher education and or Licensure given preference Required certification or license: Licenses/Degree/Certificates (MSW, LSW, LCSW, LCPC, QMHP, MHP or above).
Minimum experience required:
Two-year's experience in a substance abuse treatment, correctional, or other social services/mental health setting. The two-year requirement can include time in an approved supervised practicum or internship program. CPR and First Aid Certification preferred. Ability to work remotely (telehealth video)
