Job Description
Job Summary We are growing and are adding a new Social Worker to our hospital team. This is a hybrid/remote position but you must live in Hawaii, on any island. This is mandatory as you will need to fly into Kona or Honolulu. for meetings occasionally. The successful candidate must be licensed: BSW, LCSW or MSW. You must be empathetic, compassionate, energetic, and detail-oriented. For In this vital role, you will provide comprehensive psychosocial support to patients and their families, ensuring they receive the necessary emotional, social, and practical assistance throughout their healthcare journey. Your expertise will help improve patient outcomes by coordinating care, conducting assessments, and delivering specialized counseling services in a dynamic healthcare environment. This paid position offers an exciting opportunity to make a meaningful difference in the lives of individuals facing complex health challenges. Responsibilities Conduct thorough patient assessments to identify social, emotional, and behavioral needs in various clinical settings such as inpatient units, outpatient clinics, nursing homes, and hospice care. Provide individual, group, and family counseling utilizing techniques like cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy to support mental health and behavioral health needs. Coordinate with medical teams to develop personalized care plans that incorporate social work interventions, including crisis intervention and case management. Assist patients with discharge planning, home & community care arrangements, assisted living options, and post-acute care services to ensure smooth transitions. Maintain accurate medical documentation and records in compliance with HIPAA regulations while utilizing electronic medical record (EMR) systems for data collection and reporting. Support research initiatives by collecting relevant data related to patient care outcomes and participating in clinical research activities when applicable. Collaborate with multidisciplinary teams to deliver specialized services such as hospice care, dementia care, Alzheimer's care, PTSD care, autism support, or working with individuals with developmental disabilities. Skills Proven experience in hospital or healthcare settings with knowledge of inpatient and outpatient clinical workflows. Strong background in child & family counseling, mental health counseling, behavioral health interventions, and crisis management. Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for billing purposes, and medical documentation standards. Expertise in working with individuals with disabilities—including developmental disabilities—and providing services like special education or working with children. Excellent communication skills for intake interviews, diagnostic evaluation, group therapy facilitation, and motivational interviewing techniques. Ability to handle sensitive situations involving hospice or dementia care with compassion and professionalism. Knowledge of HIPAA compliance standards for protecting patient privacy; experience using EMR systems for documentation and data collection. Experience working with diverse populations including geriatrics (senior care), pediatrics (pediatric patients), or post-acute care environments such as nursing homes or assisted living facilities.
