This clinical social worker plays a vital role in providing specialized psychosocial assessment/evaluation pre-transplant, cellular, or gene therapy for patients and caregivers, as well as ongoing psychosocial support and multidisciplinary collaboration. This clinical social work role involves partnering with the medical team to provide collaborative care and planning for challenging and complex patient needs in preparing for cellular therapies, such as bone marrow transplant or CAR-T cell therapy. The clinical social worker will be a core member of the CAGT, providing expertise in psychosocial care for patients, caregivers and families throughout the pre-cellular therapies process, fostering adaptation and healthy coping, including advanced care planning and end of life decision making. - You will be responsible for: Pre-cellular therapy comprehensive biopsychosocial evaluation, high risk screening for psychosocial factors that may negatively impact therapy outcomes Establishing a therapeutic relationship and engaging in problem solving and planning to develop caregiver and relocation plans Patient and/or family support and counseling, including ongoing assessment of patient and caregiver adjustment, coping, and needs; provision of family-centered support and anticipatory guidance Facilitating patient and family participation in appropriate health care decision making, including goals-of-care discussions and advance care planning when appropriate Facilitation of an understanding and resolution of problems related to interpersonal conflict, mental health and substance use concerns or disorders Crisis intervention, including identification and response to high-risk situations (e.
g., suicidality, domestic violence, substance use) Liaison and referral to community agencies, including connection to financial assistance and support groups Facilitating support for insurance navigation, disability processes, and collaboration with financial counselors and community partners Participation in team meetings and educational activities, contributing psychosocial expertise to interdisciplinary care planning Coordination of care with multidisciplinary teams and support of patient transitions across the care continuum (diagnosis, treatment, survivorship, end-of-life) Advocacy for patient needs within the healthcare system to reduce barriers to care and promote equitable outcomes Accurate and timely documentation of assessments, interventions, and care coordination activities Participation in program development, quality improvement initiatives, and efforts supporting distress screening and accreditation standards Required Qualifications
- A Master's degree in Social Work from a school accredited by the Council of Social Work Education. A current license to practice social work in the State of Michigan; full LMSW is required for billing purposes. Previous experience with oncology, medical, or mental health social work. Work Schedule This is a permanent, part-time, 20 hour/week position. The typical schedule is within Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00. Work Locations This position will work out of a primary location: Michigan Medicine, Main Medical Campus, 1500 E. Medical Center Dr., Ann Arbor, MI 48109. Current offices are in University Hospital South with significant time spent in CW Mott Clinics. Underfill Statement This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification Union Affiliation This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
