Job Overview We are seeking a dedicated and energetic Telehealth Licensed Clinical Social Worker (LCSW) and Case Manager to join our dynamic mental health team. In this vital role, you will deliver comprehensive behavioral health services via telehealth platforms, ensuring accessible, compassionate, and effective care for diverse populations. Your expertise will support individuals across various age groups and backgrounds, including children, adults, and seniors, with a focus on mental health, developmental disabilities, addiction recovery, and crisis intervention. Duties Conduct thorough patient assessments, diagnostic evaluations, and clinical counseling sessions through secure telehealth systems. Develop personalized treatment plans incorporating evidence-based practices such as cognitive behavioral therapy (CBT), psychodynamic therapy, motivational interviewing, and behavioral therapy. Provide individual and group therapy sessions focused on mental health concerns including PTSD, addiction, autism spectrum disorders, Alzheimer's care, and other behavioral health issues. Manage crisis situations with prompt intervention strategies to ensure patient safety and well-being. Document all patient interactions accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations for confidentiality and data security. Coordinate case management activities including patient care planning, community resource linkage, and follow-up to promote optimal health outcomes. Collaborate with multidisciplinary teams—including medical professionals, educators in special education settings, and community organizations—to support comprehensive patient care. Participate in clinical research initiatives related to behavioral health trends or specific populations such as geriatrics or pediatrics. Skills Proven experience in psychotherapy modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, applied behavior analysis (ABA), and crisis management techniques. Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for billing purposes, and medical documentation standards. Skilled in working with children, individuals with developmental disabilities or autism spectrum disorder (ASD), and those requiring special education or Alzheimer's care. Expertise in motivational interviewing techniques to foster patient engagement and behavior change. Familiarity with EMR systems for efficient documentation and data collection; experience with clinical research is a plus. Ability to handle complex cases involving PTSD care, addiction counseling, Geriatrics, or working with individuals with disabilities across diverse settings including hospital environments or home & community care contexts. Excellent crisis intervention skills combined with compassionate social work practices that prioritize patient safety and empowerment. Knowledge of HIPAA compliance standards along with strong organizational skills for managing medical records and patient assessments effectively.
