Job Description
Summit Medical Group is seeking a Social Worker to join their team. This is a full time opportunity, working partially from home option available upon successful completion of orientation. Examples of Duties (List does not include all duties assigned) Identify SMG patients with the potential for high-risk complications and coordinate the appropriate treatment in conjunction with the patient and care coordination team. Develop and implement targeted strategies to improve overall patient health and quality of life outcomes. Refer and follow-up that provision of covered physical health, behavioral health, and/or home and community-based services are provided as a cost-effective alternative to ER admissions. Coordinate with SMG patient's primary care provider, specialists, community resources, families, and care programs, to ensure comprehensive, holistic, person-centered approach to care. Provide patients with education about the ability to create and use an advance directive. Establish collaborative relationships with referral sources, community resources, and care providers. Act as an advocate for an individual's health care needs by identifying and communicating potential needs to the patient's provider. Maintain security and privacy of patients by keeping patient care information confidential. Participate in required training and attend team, departmental, andorganizational meetings. Monitor and improve quality of services provided to patients/caregivers through ongoing participation in team and departmental quality improvement activities. Update job knowledge by participating in educational opportunities that support the advancement of Care Coordination. Participate in the orientation and training of new staff as needed. Actively participates in site-level Quality Improvement Activities. Each employe will contribute to the continual evaluation site performance as well as the implementation and measurement of improvement activities that increase the quality of care provided to patients. Performs related work as assigned. Education Bachelor's degree in Social Work or Masters Degree in Social work required. Master's degree preferred. Experience Prefer 3 - 5 years' experience in healthcare setting; 3-5 years of experience in care management, disease management, population health management, or other related health care environments. Certification/License Tennessee state license in social work preferred.
