Medical Social Worker (MSW) - HospicePRN Make a meaningful difference by supporting patients and families when they need it most. We're looking for a compassionate and dedicated Medical Social Worker (MSW) to join our Hospice team. In this role, you'll help patients and their families navigate the emotional, social, and financial challenges that come with illness while connecting them with the resources they need to improve their quality of life. What You'll Do Complete psychosocial assessments and develop individualized care plans Provide counseling, emotional support, and crisis intervention to patients and families Connect patients with community resources and financial assistance programs Collaborate with physicians, nurses, therapists, and the interdisciplinary care team Assist with advance care planning, end-of-life support, and discharge planning Maintain timely and accurate clinical documentation Why This Role Stands Out Build meaningful relationships with patients and families Make a lasting impact beyond clinical care Work independently while being supported by an experienced interdisciplinary team Flexible schedule with autonomy in managing your caseload Be part of a mission-driven organization focused on compassionate care What You Bring Master's Degree in Social Work (MSW) from an accredited program Current state licensure, if required Hospice experience preferred Strong communication, counseling, and organizational skills Valid driver's license, reliable transportation, and current auto insurance
