Job Description We are seeking a dedicated and experienced Medical Social Worker (MSW) to join our team on a PRN basis . This role is ideal for a professional seeking a flexible schedule while making a meaningful impact on patient lives. You will work closely with our clinical team to assess social and emotional factors related to patient illness and provide necessary interventions. Key Responsibilities Conduct social and emotional assessments of patients in their home environment. Assist patients and families with community resource planning, long-term care arrangements, and financial assistance applications. Provide short-term crisis intervention and counseling related to the patient's medical condition. Collaborate with the multidisciplinary team (RNs, PTs, OTs) to develop and update the plan of care. Complete and submit clinical documentation (via Kinnser/WellSky ) within our 24-48 hour compliance window . Qualifications Master's Degree in Social Work (MSW) from a school accredited by the Council on Social Work Education. Current state license to practice as a Social Worker. Minimum of 1 year of experience in a healthcare setting (Home Health or Hospice experience preferred). Strong knowledge of local community resources and Medicare/Medicaid guidelines.
