Job Description:
We are looking for a dedicated Medical Social Worker (MSW) to assess and address the psychosocial needs of our patients. The MSW will provide counseling, resource coordination, and advocacy to enhance patient well-being.
Responsibilities:
Assess patients' psychosocial needs and develop care plans. Provide counseling and emotional support to patients and families. Connect patients with community resources and services. Advocate for patients' needs within the healthcare system. Collaborate with healthcare teams to address social determinants of health. Maintain accurate documentation of assessments and interventions.
Qualifications:
Master's degree in Social Work and valid Texas MSW license. Minimum 1 year of experience, preferably in home health or medical social work. Strong knowledge of community resources and social work practices. Excellent interpersonal and crisis intervention skills. Reliable transportation and a valid driver's license. Ability to work independently and manage complex cases.
