Licensed Clinical Social Worker

Overview Seeking a compassionate and experienced Clinical Social Worker to join our hospice interdisciplinary team. The ideal candidate will have experience working with hospice or palliative care patients and families, providing emotional support, counseling, resource coordination, and psychosocial assessments during end-of-life care. Our mission is to provide compassionate, patient-centered hospice care that enhances quality of life for patients and their loved ones. Responsibilities Complete psychosocial assessments for hospice patients and families Provide emotional support, counseling, and crisis intervention Assist patients and families with coping strategies related to terminal illness and grief Coordinate community resources and referrals Participate in interdisciplinary team meetings and care planning Assist with advance directives and end-of-life planning Document visits and interventions accurately and timely Support bereavement and discharge planning services Ensure compliance with Medicare hospice regulations and agency policies Qualifications Master's Degree in Social Work (MSW) required Current Texas Social Work license (LMSW or LCSW preferred) Minimum 1 year of hospice or palliative care experience preferred Knowledge of hospice philosophy and Medicare hospice guidelines Strong communication and interpersonal skills Compassionate, organized, and patient-focused Reliable transportation and valid driver's license Benefits Competitive pay Flexible scheduling Supportive team environment Mileage reimbursement Opportunities for professional growth Meaningful work serving patients and families How to Apply Please submit your resume through Indeed or contact Prime Choice Hospice Care directly for more information.
