Hospice Medical Social Worker Full-Time Position, 37.5 hours per week

Who We're Seeking:
Are you a dedicated and compassionate Medical Social Worker (MSW) looking for a fulfilling opportunity to make a real difference in the lives of hospice patients? We are currently seeking a dynamic individual to join our interdisciplinary Hospice team, where you'll have the unique opportunity to provide direct, one-on-one support to individuals in the comfort of their homes.
Your Responsibilities:
As a member of our dedicated team, you'll: Directly engage with patients in the comfort of their own home, providing one-on-one support. Work collaboratively with a supportive interdisciplinary team, including medical doctors and caregivers, to develop personalized care plans. Maintain consistent, timely, and accurate communication with all members of the patient's health care team. Regularly evaluate each individual's progress toward their care goals, updating the care plan as needed in collaboration with the healthcare team. Participate in team meetings and collaborate with supervisors to identify challenges, and foster supportive and innovative solutions. Deliver evidence-based interventions tailored to address psychosocial challenges impacting clients' medical care.

Requirements:
Master's degree from a school of social work accredited by the Council on Social Work Education Minimum one year of progressive clinical experience in a medically-oriented setting.
