Daily Range of Responsibilities:

Conduct biopsychosocial and crisis risk assessments for adult and pediatric patients Provide individual, parental, and family counseling support Participate in treatment planning and interdisciplinary care coordination Assist patients and families with discharge planning and community resource navigation Coordinate referrals to behavioral health, rehab, long-term care, and community agencies Support patients with DCF processes, protective services, guardianship, and emergency detention resources Collaborate with physicians, nursing staff, and care teams to ensure continuity of care Maintain timely and accurate clinical documentation

Job Qualifications:
