Summary:
Assist in the development, modification, and implementation of the treatment plan for patients in cooperation with the interdisciplinary treatment team. Documents and communicates patient progress, promoting continuity of care. Participates in all aspects of the delivery of the program as appropriate.
Other information:
Bachelors degree in social work, psychology or related field. This job will be authorized 80.00 hours bi-weekly.
