A Mental or Behavioral Health Counselor works with patients to treat mental health problems. Assesses patients and assists with diagnosis, counseling and crisis intervention. Family Support Specialist 1 Are you interested in playing a pivotal role in the lives of families and children? In this role, you will support clients in achieving their individual goals by providing intensive in-home case management services. You will help families address the problems of homelessness, unemployment, mental illness, and substance use through collaboration with treatment providers, natural supports, and community resources. Essential Job Functions:
Weekly in-home visits targeted to support clients in developing and achieving goals to find housing, employment, and treatment options. Identify resources and facilitate referrals, as needed, for housing, eligible benefits, substance use, mental health, counseling, child specific services, life skills including budgeting, vocational and educational opportunities. Regular and proactive collaboration with stakeholders such as DCF, treatment providers, landlords, internal staff, and departments.
