Work Environment This role is primarily remote, utilizing telehealth technologies to connect with patients effectively. Counselors will operate within a supportive and collaborative virtual environment, working closely with primary care providers and other healthcare professionals to deliver integrated care. Key Responsibilities As a Mental Health Counselor at Big Leap Family Guidance Centers, you will: Assist primary care providers in recognizing and managing mental health and psychosocial issues, enhancing the overall care of patients. Conduct thorough patient intakes that focus on diagnostic and functional evaluations, subsequently advising primary care providers on treatment recommendations. Provide brief, focused interventions to clients requiring mental health services, ensuring that care is tailored to individual needs. Evaluate patient needs for specialty mental health referrals and collaborate with other providers to ensure seamless transitions in care. Actively monitor patient progress and adjust treatment plans as necessary, including developing relapse prevention strategies for at-risk patients. Document treatment progress meticulously and maintain accurate clinical records to comply with agency protocols. Support patients in understanding their treatment options and promote self-management skills for their recovery journey. Provide insights and consultation to clinic management on behavioral services and program evaluation. Assist patients in adhering to medical treatments prescribed by primary care providers, including strategies to manage medication side effects. Patient Population / Client Focus Your work will focus on a diverse patient population, including individuals facing a spectrum of mental health challenges such as anxiety, depression, trauma, and other behavioral health issues. You will be instrumental in delivering care that respects each client's unique circumstances, strengths, and goals, while ensuring the high standards of confidentiality and trust that underpin all client interactions. Qualifications The ideal candidate will possess: A Master's Degree in Clinical Counseling or a related field. A current Licensed Professional Counselor (LPC) license in the state of Connecticut. In-depth knowledge of mental health assessment techniques, treatment planning, and behavioral therapy methods. Strong ability to work collaboratively as part of a multidisciplinary team, maintaining effective communication with colleagues and clients. Competence in maintaining thorough clinical documentation and preparing related reports with meticulous attention to detail. Exceptional judgment to make independent decisions based on professional insights and ethical considerations. Preferred Experience While not required, the following qualifications are preferred: Proficiency in both English and Spanish to engage effectively with a broader client base. Experience working within a Federally Qualified Health Center (FQHC) or similar community health settings. A strong understanding of community resources and networks that support patient care. Compensation and Benefits Big Leap Family Guidance Centers offers a competitive salary range of
USD 87,000 - 105,000
