Job Summary and Qualifications The Intake Licensed Mental Health Professional III facilitates high quality, patient-centered care by providing psychiatric treatment services to patients and their families. The Mental Health Professional III performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy. What you will do in this role: You will assess, plan, intervene and evaluate the care of psychiatric patients. You will instruct patients and families regarding treatment instructions Demonstrates advanced clinical skills and ability to plan and facilitate individual, group and family psychoeducational and process groups. Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis. Collaborates with the multidisciplinary team to assess and coordinate patient's discharge planning needs and readiness for discharge. Provides guidance on complex issues. Conducts post-discharge follow-up as indicated. Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.

Requirements:

Master's degree in a Human Services related field (Psychology, Social Work, Mental Health, or Counseling Advanced educational preparation or experience in the area of clinical practice and/or management Currently licensed as one of the following professionals in the state(s) of practice, according to law and regulation: Licensed Clinical Social Worker (LCWS) Licensed Professional Counselor (LPC)

• Licensed Marriage and Family Therapist (LMFT)
• Licensed Mental Health Counselor (LMHC)
• Licensed Independent Social Worker (LISW, LICSW)
• Must be authorized by the state to provide outpatient psychiatry treatment services to Medicare beneficiaries.
