Category Health Care Apply Description Regular Full Time Status:
Non-Exempt/Hourly Reports to: Clinic Administrator Medical, Dental, Vision, 401K, Vacation, Paid Holidays Essential Duties and Responsibilities (This is not an exhaustive list of all duties/responsibilities. Other duties may be assigned) Provide on-site rehabilitative services for those experiencing severe and persistent mental illness and/or substance use disorders. Assist clients in obtaining and implementing skills for coping with issues such as trauma, familial and/or peer conflict, mental health disorders, substance abuse, grief and loss, relational issues and community re-engagement. Provide individual, couple, family and group counseling as assigned. Assess patients continuing care needs via chart review, direct interview, and completion of the Treatment Needs Assessment Collaborate with other clinic staff in the development and implementation of appropriate treatment plans and interventions. Generate appropriate medical chart documentation. Assist patients in the completion of the discharge planning worksheet and provide collaborative linkages with other community programs and services. Schedule and coordinate aftercare appointments/activities Communicate via a written report- daily discharge planning activities.
