As a trusted behavioral health organization committed to providing care in the community since 1955, we offer an array of comprehensive services for youth, adolescents, and adults. Our Intake Team acts as the first contact for new clients by returning calls on our Intake Line and meeting with clients to get started. Our Assessment Team provides Mental Health Assessments, Reassessments, and Individual Treatment Planning. Primary Responsibilities Accept assessments assigned by Intake/Assessment Supervisor. Complete assessments as scheduled in a timely manner. Evaluate individuals for appropriate services and make all necessary referrals, both within and outside the agency. Assist in developing services to a caseload of clients, who have a mental illness which is serious and persistent and/or substance use needs. Assist individuals who live independently in the community via linking and referring to housing, food and transportation resources, applying for entitlements, providing counseling and life skills training and advocating for the individual's needs. Complete treatment plans, annual re-assessments, and IATPs for a caseload of clients, ASAM, DLA and LOCUS assessments as required. Participate in developing goal-oriented ITP's that are strength-based and recovery oriented on all assigned clients at least every six months or as the client's plan requires. For substance-use-related programming, provide client orientation which includes the goals of the treatment program; rules governing client conduct and infractions that can lead to disciplinary action or discharge from the program; the hours during which services are available; financial costs of the program and client rights. Provides minimum quarterly outreach and supports to a caseload of individuals engaged in psychiatry services, includes ensuring IATP is completed prior to psychiatry appointments and that insurance or other payor information is up to date. Follow and remain knowledge of SUPR rules, and Medicaid regulations, agency policies and procedures and Federal Confidentiality Regulations. Performance Requirements Demonstrate knowledge of documentation standards and capacity to meet documentation expectations. Ability to assess, diagnose, interpret and integrate both verbal and non-verbal client input during session Ability to establish rapport with patients and interact constructively with others in the workplace. Ability to focus client on individualized treatment plan and goals. Knowledgeable regarding the classification of current psychotropic medications and Medication Assisted Recovery Give information in a clear and understandable fashion and respond constructively to appropriate client questions and concerns. Education and Experience Master's Degree in human service field and must hold clinical certification as a Certified Alcohol and Drug Counselor (CADC) from the Illinois Alcoholism and Other Drug Abuse Professional Certification Association (IAODAPCA) or be licensed as an LPC, LSW, LCPC, or LCSW. One-year clinical experience Willingness to work a flexible schedule, if needed Valid Illinois driver's license, insurance, and automobile and willingness to transport client when necessary.
