  • Albany Care Albany Care, a Specialized Mental Health Rehabilitation Facility (SMHRF), in Evanston, IL, has Immediate Openings for Mental Health Professionals (MHPs) to join our team.
These are Full Time Positions with 8 Hour Shifts. Starting Rate of up to $28 per hour, depending on Benefits Selection!

Mental Health Professionals (MHPs) work in conjunction with an interdisciplinary team, offering recovery and rehabilitation support and services to those who live in the facility. Responsibilities of the Mental Health Professional (MHP) Collaborate with consumer and treatment team to complete assessments and develop therapeutic treatment plans Implement therapeutic care plans and activities in a compassionate and caring manner Recognize, note, and inform treatment team when a consumer is not at their baseline of symptoms and behaviors Lead groups and hold one-on-one sessions including engagement with evidenced based approaches Collaborate with community agencies to coordinate care, including the Moving On Program Crisis management and intervention
  • Effectively manage a caseload of up to 32 individuals Participation in creation of discharge plans and recommendations for other levels of care In Addition to a

Requirements:
We Will Consider Candidates Who Have at least One of the
Following Qualifications:
Bachelor's Degree in Human Services Field (counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, etc.) Bachelor's degree in other field + 2 years of supervised clinical experience in a mental health setting Licensed Practical Nurse (LPN) Certificate of Psychiatric Rehabilitation from an Illinois Department of Human Services (DHS) approved program + High School Diploma/GED + 2 years' experience providing mental health services Certified Recovery Support Specialist (CRSS) Licensure through the Illinois Alcohol and Other Drug Abuse Professional Certification Association (IAODAPCA) Occupational therapy assistant + 1 year experience in a mental health setting High School Diploma + 5 years of supervised clinical experience in mental health or human services
