We are currently accepting applications for QMHP Sharon Healthcare Woods is a large residential facility which serves a population of mentally ill adults with diverse needs. As the QMHP you will be required to work with Administration and other social service staff to ensure that the facility's Psychiatric Rehabilitation Services meet resident needs as well as satisfying the standards promulgated by National, State and local regulatory agencies.
Principal Duties:
Supervise Psychosocial Rehabilitation Service Coordinators to: Assist in development of an Interdisciplinary Treatment Plan for each resident Review and revise (in conjunction with the PRSC) as necessary the psycho-social problems, goals and interventions within the Treatment Plans Respond to concerns and questions and act quickly to resolve these in a satisfactory manner Review and explain facility policies, responsibilities and resident rights to the resident and/or family when requested Ensure that residents/family and/or representatives are aware of the resident care conference and how individuals can participate in this meeting if they so choose Assist resident and family, and others as appropriate, in coping with the effects of illness, treatment, or stay in the facility Write progress notes in resident records as needed. Personnel Specifications:
Requirements Licensed social worker (LSW) with a master's degree and two years at least 2 years' mental health experience Master's Degree from an accredited college in social services or related field, i.e. psychology, rehabilitation counseling Licensed professional counselor with a master's degree with two years mental health experience RN with one year mental health experience or an RN with a master's degree in psychiatric nursing Occupational therapist with one year of mental health experience Individual with master's in counseling and guidance, rehabilitation counseling, family therapy or related field who has successfully completed a practicum or internship that included 1,000 hours of supervised direct service in mental health setting Experience supervising social service personnel Thorough knowledge of clinical practice, medical and public health settings; working knowledge of social and health resources of the community Must be able to exercise independent judgment where procedures cannot be standardized Sharon Healthcare is an equal opportunity employer
