Position Purpose:
Provides mental health rehabilitation services to clients and families to decrease hospitalization, crisis episodes, and to increase community functioning for the client to achieve rehabilitative, resiliency and recovery goals. Behavior Health Care Services will be delivered full-time on site at Good Shepherd YMCA Rockford, IL 61103.
Job Requirements:
Bachelor's Degree in Psychology, Counseling, Social Work, Addiction Studies or other related field, or equivalent experience and training Must qualify as a MHP per Illinois Department of Mental Health Rule 132 Excellent Possess excellent written and oral communication skills Computer proficiency required Ability to navigate and complete documentation in the electronic healthcare record Ability to accurately complete required documentation within the prescribed time frames Skilled in conducting individual, family, and group counseling Ability to present and coordinate lectures for clients, parents, staff, or community resources Self-starter with the ability to work independently and collaborate within a team environment Valid driver's license and ability to meet Agency insurance requirements (designated positions) Must have own transportation (designated positions) Belief in the mission and vision of
Rosecrance Essential Responsibilities:
1. Work hours prescribed by, and any additional hours deemed necessary for the responsibilities assigned. 2. Work onsite at the assigned location or at an alternative location as deemed necessary by leadership. 3. Integrate into the team and actively participate in collaborative efforts. 4. Identify youth who may require behavioral health services. 5. Send a group note via email for each facilitated group session on a weekly basis to Rosecrance supervisor. 6. Complete a sign-in sheet for youth attending group sessions and email copy of sign in sheet to Rosecrance supervisor. 7. Email a sign-in sheet to document hours worked on-site. 8. Notify leadership when leaving the work location during work hours and provide an estimated return time for transparency. 9. Complete all necessary documentation within the appropriate timelines. 10. Undertake primary case management responsibilities for designated clients. 11. Conduct individual, group, and family counseling and educational sessions for clients and/or family members. 12. Ensure that client service records are maintained consistent with rules and regulations. 13. Maintain communications with all staff (e.g., admissions, process, and discharge plans). 14. Coordinate services from various community resources to support client needs effectively. 15. Utilize a variety of treatment concepts, evidence-based practices and service modalities when providing care. 16. Present workshops, lectures and training to clients, families, and staff and community groups as needed. 17. Be familiar with court proceedings and available to make court appearances on behalf of Rosecrance clients. 18. Provide individual services such as short-term therapeutic support, interventions, and immediate crisis stabilization to participant and/or family. 19. Facilitate group services focused on clinical and early intervention strategies, utilizing an evidence-based curriculum centered on overcoming obstacles. 20. Milieu management services with in-vivo interventions as a part of assigned location programming. 21. Training, education, and consultation on behavioral health issues with assigned location staff and personnel. 22. Provide ongoing case management, coordinating and communicating effectively with stakeholders in the assigned location, clients and parents. 23. Develop and implement programs focused on overcoming obstacles to ensure successful outcomes. 24. Serve as a member of the Clinical Team and participate in all team meetings and activities. 25. Exercise confidentiality in keeping with the professional Code of Ethics and within the framework of the law. 26. Deliver exceptional customer service consistently to every customer. 27. Serve as a role model and demonstrate positive guest relations in representing the agency. 28. Assume other related responsibilities as delegated by leadership.
Schedule:
8-hour shifts
Work Hours:
Summer hours are Monday through Friday 8:30am-5pm and during the Rockford Public Schools school year Monday through Friday 11:00am to 7:30pm

Behavior Health Care Services will be delivered full-time on site at Good Shepherd YMCA Rockford, IL 61103.
Onsite Compensation & Rewards Base Pay:
Starting at $50,835.20 year ( pay is based on education, experience, and credentials ) Professional Certification (e.g., CADC, CRSS, CPS): $1,500 annually Clinical Licensure (e.g., LPC/LSW/LMSW, LCPC/LCSW/LMHC): $1,800 - $5,000 annually
