We are seeking a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Health Service Provider in Psychology (HSPP), or Licensed Marriage & Family Therapist (LMFT) to join our growing behavioral health team serving patients residing in Skilled Nursing Facilities (SNFs) throughout Indiana. This position allows you to provide evidence-based therapy while collaborating with interdisciplinary healthcare teams to improve the emotional and behavioral well-being of patients with a wide range of mental health and substance use disorders . Experience working with the geriatric population is highly preferred.
Schedule:
Monday - Friday, 8:30am - 5:00pm What You'll Do Conduct comprehensive biopsychosocial assessments for new patients. Develop individualized treatment plans based on clinical presentation and medical necessity. Provide evidence-based individual psychotherapy through secure telehealth platforms. Deliver trauma-informed, patient-centered care to adults residing in Skilled Nursing Facilities. Evaluate patients for depression, anxiety, cognitive concerns, trauma, substance use disorders, grief, and other behavioral health conditions. Conduct suicide risk assessments and crisis intervention when clinically appropriate. Coordinate care with physicians, nursing staff, social workers, rehabilitation teams, and other interdisciplinary providers. Participate in treatment planning and care coordination meetings with facility staff. Maintain timely, accurate, and compliant clinical documentation, including assessments, treatment plans, progress notes, and discharge summaries. Ensure services meet payer, regulatory, HIPAA, and Indiana licensing requirements. Provide post-discharge behavioral health follow-up when appropriate. Participate in clinical supervision, quality improvement initiatives, and team meetings. QualificationsRequired Active, unrestricted Indiana license as one of the following:
LMHC LCSW LMFT HSPP
Master's degree or higher in Counseling, Social Work, Psychology, Marriage & Family Therapy, or related behavioral health field. Ability to independently provide psychotherapy and behavioral health assessments. Strong clinical documentation and organizational skills. Comfortable utilizing electronic health records and telehealth technology. Preferred Experience providing therapy to the geriatric population. Previous experience working within Skilled Nursing Facilities, Long-Term Care, rehabilitation, or medical settings. Experience treating individuals with co-occurring mental health and substance use disorders. Familiarity with evidence-based treatment modalities including CBT, DBT, Motivational Interviewing, Solution-Focused Therapy, and Trauma-Informed Care. Experience collaborating within interdisciplinary healthcare teams. Why Join Us?
