Cumberland County seeks a Medication Assisted Treatment professional to support residents with substance use disorders through evidence-based care. In this role, you will coordinate and deliver MAT services, complete assessments, develop treatment plans, and monitor medication adherence in collaboration with physicians and behavioral health staff. You will document care in EHR systems, ensure compliance with state and federal regulations, and connect clients to community resources. Responsibilities Coordinate and deliver medication-assisted treatment (MAT) services for individuals with substance use disorders Conduct clinical assessments and develop individualized treatment and medication plans Monitor medication adherence, response, and side effects, adjusting plans in collaboration with prescribing providers Document all services and treatment activities in the electronic health record (EHR) accurately and timely Ensure compliance with federal, state, and county regulations and protocols governing MAT programs Provide education and counseling to patients and families on medications, recovery, and community resources Collaborate with physicians, nurses, counselors, and case managers to support integrated care Track outcomes and assist with data reporting to support program evaluation and improvement Coordinate referrals and link clients with supportive services, including mental health, housing, and social support Participate in community outreach, team meetings, and training to support program development and best practices Required Skills Medication-assisted treatment protocols (e.g., buprenorphine, methadone, naltrexone) Clinical assessment and treatment planning Electronic health records (EHR) documentation Knowledge of federal and state regulations for MAT programs Case management and care coordination Substance use screening and monitoring tools Patient and family education Data collection and program reporting Interdisciplinary team collaboration Crisis intervention and de-escalation
