POSITION SUMMARY
: The Family Support Specialist, assigned to the Wayne County HFA program will be responsible for conducting culturally appropriate community outreach activities including lay health education and access to services related to grant-specific community-based programs. The Family Support Specialist will provide ongoing, in-home services to a caseload of clients (pregnant women; newborn infants; women whose infants are at high risk for morbidity/mortality; child abuse/ neglect; documentation of substance abuse and/or infant/ children at risk for foster care placement) based on the standards established by the Healthy Families America Program. Case finding/ client recruitment is done in the Wayne County MI community. Follow-up home visits are an essential, ongoing aspect of this position.
ESSENTIAL FUNCTIONS
Provide comprehensive home visiting services to eligible individuals and families in service of the area. Participate in case finding and engage members of at-risk populations who may face obstacles to a healthy pregnancy and a positive pregnancy outcome. Demonstrate culturally competent strategies to educate and motivate clients regarding healthy living and health-seeking relationships or therapeutic alliances related to prenatal, postpartum and continuing comprehensive care. Link client's-to public health and other identified sources, based on identified risk. Role-model effective behavior in interactions with family, official agencies, and health care providers. Maintain a home visit caseload with the goal of reaching child's third to fifth birthday. May require you to be flexible to accommodate family schedules. Maintain confidentiality of protected health information. Must have the ability to communicate effectively with public stakeholders. (DHS and CPS). Maintain current paper and electronic records of all client contacts using correct, Standard English. Enter data into electronic databases from assessment instruments as assigned. Alert program staff regarding client needs for other social service agency referrals, as indicated. Compile, prepare, and submit weekly and monthly reports in a timely manner. Participate in weekly case conferences, group counseling, and client evaluations as necessary. Attend appropriate community resource meetings and training as assigned. Participate in personal and professional development of self and others. Perform related duties as assigned/required
JOB QUALIFICATIONS
Minimum one to two years of paid, documented experience within a public health; DHS; CPS or health care setting performing outreach or other community-based functions such as home visits, or case management as part of a team, and/or prior community outreach experience with specific experiences within the maternal/child at risk population is preferred. Valid Driver License and a reliable transportation is required. Computer literacy is required. Ability to speak other languages fluently is preferred. Ability to work with people from diverse backgrounds, lifestyles, medical conditions, and experience is required.
EDUCATION
Bachelor's degree in Social Work; Counseling; Early Childhood Education or an acceptable related field from an accredited college or university.
