Bachelor's Degree or Higher:
Starts at $24.69
Schedule:
Full-Time:
Monday - Friday 7:30 AM - 4:00 PM with rotating on call evenings/weekends.
OPPORTUNITY
Central Minnesota Mental Health Center (CMMHC) has a rewarding opportunity to serve as a Behavioral Health Practitioner within our St Cloud Assertive Community Treamment (ACT) team. This position provides a continuum of recovery oriented, skill building interventions under the supervision of a Mental Health Professional.
WHAT IS ASSERTIVE COMMUNITY TREATMENT?
Assertive Community Treatment (ACT) is an intensive, non-residential mental health service for persons with serious mental illness. These individuals commonly experience multiple barriers to successful functioning, including co-occurring substance abuse or dependence, homelessness and unemployment. ACT teams help individuals treat and manage their mental illnesses and develop the skills they need for life in the community of their choice.
ACT services are:
Provided by multidisciplinary team A blend of treatment, rehabilitation, therapeutic, and supportive services Directed to persons with a serious mental illness who require intensive servicesOffered on a time-unlimited basis and available 24 hours per day, 7 days per week, 365 days per year.
DUTIES AND RESPONSIBILITIES
Implement services in the treatment plan for each recipient under clinical supervision.

Drive to client homes or various locations within the community where services are provided, and transport clients.

Establish therapeutic relationships with assigned clients. Provide medication monitoring and education regarding health maintenance to encourage a healthy lifestyle. Complete assessments and maintain client documentation. Coordinate services with other programs and providers.
