HOW YOU'LL MAKE AN IMPACT
The Adult Rehabilitative Behavioral Health Practitioner is responsible for home and community-based rehabilitative mental health services to individuals diagnosed with a mental illness and co-occurring disorders. Instructs, assists, and supports recipient in area's such as: interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment-related skills, and transition to community living services. Ensures immediate and appropriate interventions are provided in response to changes in mental status, symptoms or behavior that may put the recipient at risk, changes must be made in the Individual Treatment Plan. Establish therapeutic relationships with assigned recipients and meet with each client as planned; Must have a good understanding of therapeutic boundaries. Reflects a philosophy that is respectful and appreciative of cultural and other differences that may be encountered when interacting with co-workers and in serving recipients. Involves driving to client homes or various locations within the community where services are provided, and occasionally transporting recipients per supervisor direction. Complete initial and ongoing training requirements defined for the position to meet required standards. Prepares DLA-2
  • assessments, Mental Health Information System (MHIS) reporting and develop individual treatment plans for assigned recipients; complete individual crisis plans annually; review and revise each document as required.
Develops and maintains recipient files according to the requirements of Minnesota Statute 256B.0623; document all services provided accurately, completely and within required deadlines. Participates in discharge planning process for recipient being discharged from an Intensive Residential Treatment Services facility or inpatient hospital setting when Transition Community Living (TCL) has been authorized. Responsible for care coordination and ongoing participation in collaborative treatment planning meetings, whether it involves coordination within CMMHC's programs or with another entity identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination. Participate actively in staff meetings and supervision.
WHAT YOU'LL NEED
Qualify as a Behavioral Health Practitioner (BHP) 4,000 hours working with those with mental health, developmental disabilities, or chemical dependency diagnosis: which can be part
  • time over several years or 2 years full time.
Or Holds a bachelor's degree in behavioral sciences or related fields and completes a practicum or internship or employment that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields (2,000 hours). Or Holds a master's degree or another graduate degree in behavioral sciences or related fields. Clear DHS background study Valid Driver's license and acceptable driving record. Willingness to become medication certified
