POSITION SUMMARY
The Behavioral Health Consultant provides a range of psychological screening, assessment, diagnostic, and brief therapeutic interventions for patients referred for behavioral health treatment. The Behavioral Health Consultant works in a primary care medical setting and serves to integrate physical health with behavioral health utilizing the Patient-Centered Medical Homemodel of care.KC CARE CULTURE CODE KC CARE

Clinical Care Provides on-site behavioral health screenings, assessments, consultations, and referral for patients in a primary care medical setting. Provides brief and short-term behavioral intervention and crisis intervention as needed Implements clinical pathways and protocols for treatment of selected chronic conditions. Documents appropriate health records in the electronic medical record. Follows up to determine reliability and validity of treatment used; makes recommendations regarding modification to services and service delivery. Works collaboratively with Health Center staff and local human service organizations, treatment centers, and other community-based organizations to facilitate appropriate care coordination, information, and referrals. Works collaboratively with members of their clinical team and the Behavioral Health Department staff to support patient engagement following referral from primary care. Program and Quality Management Participates in the review of clinical issues and behavioral health program policies and procedures. Participates in quality improvement projects as requested, including researching client data/trends, utilizing data measures to prioritize client contact, and assisting in streamlining internal procedures. Completes quarterly peer reviews and assessment of chart documentation and adherence.MINIMUM REQUIREMENTS
Master's degree in social work, psychology or counseling degree Licensed in the State of Missouri as a Ph.D., LPC or LCSW Six months of directly related experience which may include internship and/or practicumPREFERRED REQUIREMENTS
Experience working with clients who have chronic health conditions, and/or substance abuse conditions Experience in providing both group and individual therapyWORKING CONDITIONS AND DEMANDS
