JOB SUMMARY
Provide direct support to adults in community-based (group home) transitional housing, helping them build skills, maintain stability, and work toward personal goals. Offer on-site supervision, counseling, advocacy, and goal-focused support. Help consumers build daily living skills using evidence-based practices, including: Personal hygiene Housekeeping, laundry, meal planning, shopping, and meal preparation Budgeting, money management, and banking Community involvement and meetings Recreation and social skills Accessing benefits and entitlements Medical case management (appointments, prescriptions, follow-ups) Employment and educational support Providing safe transportation in an agency vehicle Respond to crises and contact appropriate staff or authorities when necessary. Communicate important updates to the Supervisor and ensure smooth handoff to the next shift. Supervise and track medications in line with agency guidelines. Assist with collecting service fees. Maintain required safety standards, including fire drills, inspections, and reporting facility issues. Help keep the environment clean, safe, and secure at all times.
Qualifications:
Bachelor's degree in mental health, social work, psychiatric rehabilitation, nursing, or a related discipline; OR a combination of one or more years of college plus work or life experience together equaling four years; OR a High School diploma and four years of related mental health experience. Must possess a valid driver's license, with a good driving record. One year of mental health experience, preferred . VEVRAA Federal Contractor. Request Priority Protected Veteran Referral.
