Job Type:
Independent Contractor (1099) | Full-Time Caseload We are seeking compassionate and dedicated Social Workers and Mental Health Clinicians to provide community-based services throughout New York City. Providers will work within their preferred borough—Queens, the Bronx, or Brooklyn—with no travel required between boroughs. Caseload averages approximately 20 clients per month. Responsibilities Conduct in-home psychosocial assessments and evaluations Develop individualized service and treatment plans Provide counseling, support, and crisis intervention services Maintain timely and accurate clinical documentation Coordinate care with families, caregivers, and community providers Ensure compliance with agency and state documentation requirements Requirements Active New York State LMSW, LCSW, LMFT, or LMHC license Valid driver's license and reliable personal vehicle required Must be comfortable driving throughout your assigned borough, including Queens if selected Ability to travel to clients' homes within your assigned borough Strong communication, documentation, and organizational skills Experience providing community-based or home-based services preferred Bilingual candidates and applicants with lived experience of disability are strongly encouraged to apply. #ind456
