Job Description

TITLE:
COURT-BASED MENTAL HEALTH NAVIGATOREDUCATIONAL REQUIREMENTS
Master's degree in Social Work, Mental Health Counseling, Psychology, or related behavioral health field.
LICENSURE/CERTIFICATION
Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Psychologist, consistent with New York State eligibility requirements.
EXPERIENCE/SKILLS
1. Minimum of three years of experience providing clinical services to justice-involved individuals or similar populations, including assessment and diagnostic work. 2. Experience conducting biopsychosocial assessments, formulating diagnoses, completing integrated mental health/substance use evaluations, and preparing written recommendations. 3. Familiarity with court systems, criminal justice processes, and behavioral health systems of care in Broome County. 4. Strong knowledge of co-occurring disorders, treatment modalities, and referral processes. 5. Experience working collaboratively with interdisciplinary partners such as courts, attorneys, care management agencies, treatment providers, and law enforcement. 6. Skilled in risk assessment, including suicide risk, overdose potential, and safety planning. 7. Ability to navigate complex service systems and reduce barriers to treatment engagement. 8. Strong documentation skills with the ability to meet Medicaid, OASAS, and program-specific billing and reporting requirements. 9. Demonstrated cultural competence and ability to work with diverse populations.
DUTIES:
Under the direction and supervision of the Clinical Director, the Court-Based Mental Health Navigator will: Conduct initial screenings for individuals referred through the Broome County court system. Complete comprehensive clinical assessments and integrated evaluations, including clinical impressions/diagnoses and documented recommendations for treatment or supports. Identify behavioral health needs, risk factors, barriers to care, and appropriate levels of intervention based on assessment findings. Develop written recommendations and referral plans derived from clinical evaluation results. Facilitate rapid linkage to appropriate mental health, substance use, and community supports, including ongoing care management when indicated. Follow up with individuals and referral agencies to confirm successful engagement and address barriers to service entry. Maintain an up-to-date knowledge base of local treatment, support services, and referral resources. Communicate and coordinate with court personnel, attorneys, law enforcement, treatment providers, and other system partners to support voluntary engagement in services. Provide education to court and justice system partners regarding behavioral health resources and Navigator services. Enter required clinical documentation and data into ACBC's EMR and any Broome County/partner reporting systems in a timely and compliant manner. Ensure all documentation meets ACBC standards and complies with Medicaid, OASAS, and confidentiality regulations (including
HIPAA/42 CFR
Part 2). Participate in required training, supervision, and case consultation as directed. Attend meetings with county partners and key stakeholders as needed to support program implementation, quality, and continuous improvement. Perform additional duties as assigned by the Clinical Director or program leadership.
