Job Description
Job Title:
Clinician Who we are Stepping Stone is a behavioral health agency specializing in long-term care, servicing the geriatric population residing in nursing homes and assisted living facilities throughout the state of Ohio. Our experienced and dedicated team assist the residents transition into a long-term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues. Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and/or assisted living facility, ensuring that each client's unique mental health and substance use needs are met with compassion and expertise.Responsibilities:
SOME TRAVEL REQUIRED
(PAID)- Conduct thorough assessments
- biopsychosocials and give proper diagnosis.
- Develop customized care plans for each client, considering their unique circumstances and mental health requirements.
- Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively
- Regularly assess clients' progress and adjust care plans as needed.
- Keep track of treatment outcomes and make necessary modifications
- Document treatments, interventions, and services provided.
- Engage in therapeutic conversations with clients to address emotional and behavioral issues.
- Facilitate TBS groups and IOP groups for MH and SUD.
- Facilitate psychotherapy groups.
- Complete Treatment plans and ASAMs.
- Provided 1:1 individuals to clients inside of the nursing home.
- Use evidence-based techniques to support clients in coping with challenges.
Education and other requirements:
- Must be 21yrs or older.
- Must have a valid drivers license
- Ohio Medicaid billing Number
- NPI Number
- Must have a minimum of an associates degree and Ohio License to practice.

$62,000.00- $70,000.
