Licensed Diagnostic Assessor Personal Growth Counseling Services Cleveland, OH Job Details Part-time 1 day ago Qualifications Microsoft Word Electronic health records (EHR) management Driver's License Full Job Description 10-15 hours per week availability at a minimum Join Personal Growth Counseling Services as a Diagnostic Assessor Be part of a committed team supporting youth and families through expert clinical assessment and thoughtful care planning. In this role, you'll conduct diagnostic evaluations, assess individual and family needs, and recommend appropriate treatment options to promote long-term wellness. Services may be delivered in a variety of settings, including the community, client homes, or program sites to meet families where they are.
Key Responsibilities:
Conduct comprehensive mental health assessments for youth and families using DSM-5 criteria. Provide clinical insights and treatment recommendations based on diagnostic findings. Deliver services in-person across community-based settings, including homes and schools. Maintain timely, accurate, and professional clinical documentation in the EHR system per PGCS policies. Collaborate with cross-functional teams and community partners to ensure coordinated care. Uphold strict client confidentiality and ethical standards in every interaction. Maintain flexibility and availability for scheduled assessment appointments. Submit weekly updates to the Clinical Services Director. Participate in team meetings and agency initiatives that support the mission of PGCS. Perform other duties as assigned in alignment with agency goals and values.
Qualifications:
Bachelor's or Master's degree in Counseling, Social Work, or a related behavioral health field. Current Ohio licensure (LSW, LISW, LPC, LPCC) is required. Minimum of 2 years' experience in diagnostic assessment or related clinical work. Strong computer skills; proficiency in Microsoft Word and EHR systems. CPR/First Aid and de-escalation training (e.g., CPI or NCI) must be completed within 90 days of hire. Must have a valid driver's license and an acceptable driving record.
