Job Description

Are you looking for an opportunity to advance your career while working with an extraordinary team? At Merakey, we put heart and soul into everything we do. We are seeking a Co-Occurring Therapist to join our Assertive Community Treatment (ACT) team supporting Carbon, Monroe, and Pike Counties.
Rate:
$26.94 an hour + mileage reimbursement
Location:
 912 Main Street, Stroudsburg, PA 18360 Position Details Collaborate with psychiatrists, primary care providers, and ACT team members to support integrated treatment planning for individuals with co-occurring mental health and substance use disorders, including coordination of medical, psychiatric, and community-based care. Provide ongoing counseling, education, and clinical support to individuals and families, including psychoeducation on substance use disorders, participation in treatment planning, and assistance with linking to community resources and services. Deliver community-based ACT services, including home visits and field-based support, while contributing to clinical documentation, staff meetings, and, when needed, court-related reports or testimony. Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management. Services involve weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks.
