Job Overview For over 51 years, Elder Care Services has been fulfilling our mission to improve the quality of life for seniors and their caregivers in the Big Bend. Elder Care Services currently serves as the Lead Non-Profit Agency for in-home services in Leon and the surrounding counties. Our comprehensive services provide community based, compassionate care for a broad spectrum of seniors. For active seniors, we provide an avenue to stay engaged in the community through volunteering placement. For families beginning the aging process, we are an advocate and trusted resource. For frail and homebound seniors, we continue to provide a variety of services that promote independence and safety while individuals age in place. We are looking for a helpful, compassionate, and highly motivated Counselor to join our team . As our Counselor, you will have the chance to improve the lives of seniors in the Big Bend We will provide an annual salary starting at $48,750 and a full benefits package (see below for details). We are specifically looking for a self-starter who enjoys staying busy, works well in a fast-paced and high volume atmosphere, and has a passion for service. If this describes you, please read on! Responsibilities Evaluate client and caregiver referrals to determine appropriateness for gerontological counseling. Work closely with case manager to determine client/caregiver needs. Provide client/caregiver with information regarding the counseling program. Complete client assessment and paperwork at first face to face contact. Involve the client in the care plan development. Develop care plan and assure care plan approval is obtained from supervisor. Assure counseling care plan coincides with client HCBS care plan. Complete counseling according to care plan. Review care plan on a quarterly basis. Complete case notes and assure billing of counseling services in timely manner. Advocate for the needs of clients and caregivers. Report progress towards counseling goals to case manager. Maintain confidentiality and security of Protected Health Information (PHI) according to HIPAA guidelines. Perform other duties as assigned by the Clinical Director and/or the President/CEO. Qualifications Master's degree in social work, psychology, mental health, marriage and family or related field is required LCSW, LMHC, or LMFT is preferred. Excellent computer, written and verbal communication skills. A reliable personal vehicle available and a good driving record, driving within county limits is required. Available to work hours beyond the usual workday of 8:00 a.m. to 5:00 p.m. when necessary.
