Division/Department/Program:
Horizon Respite View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, as well as other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support. The Housing Support Coordinator will provide Housing Coordination Services to individuals involved in various Housing Programs. The initial goal is to assist adult individuals with securing and maintaining stable housing.
Duties & Responsibilities:
Engage in assertive outreach with individuals to build rapport and quickly move participants out of homelessness. Provide case management services to address enrolled clients' barriers to housing. Conduct a minimum of 1 home visit per week/per client within their community setting or within client's current residence. Work with other service providers and staff in support of effective consumer treatment, linkage, communication, and continuity of service. Maintains, completes, and submits monthly
CONNECTS, CRA
Checklists, Housing Logs, and Housing plans for participants. Write progress notes and other supportive documentation that is compliant with program, payer, and agency requirements. Keeps records on each consumer that complies with all VPH and other regulatory authority standards for content and maintenance. Use personal vehicle for consumer transportation needs when agency vehicle is unavailable. Represents the site at meetings, tours and visits, inspections, audits, and evaluations. Stays up to date on training as required by VPH and all other regulatory authorities. Other related duties as assigned.
Minimum Qualifications:
Bachelor's degree in counseling or a related field from an accredited college or university AND Two (2) years of job-related experience OR One (1) year of experience at the lower-level Behavioral Health Counselor 1 (HCP100) or position equivalent.
Note:
Some positions may require certification or licensure.
Preferred Qualifications:
Master's degree in a related field from an accredited college or university. Certification as an addiction counselor from the Georgia Addiction Counselors Association (GACA) or equivalent certification Two (2) years working in homeless services. Two (2) years working in rapid rehousing Familiarity with individualized support plan development and community resources
Requirements/Competencies:
Must be able to lift 20 pounds, Requires long periods of sitting/standing, Must be 21 years of age, Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy. Candidates for selection must pass a criminal background check (including fingerprinting). Must pass a pre-employment drug screen and subject to random drug screens. Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities
