HIRING 1-2 PART TIME SERVICE COORDINATORS TO PROVIDE IN-HOME SERVICES COUPLED WITH WORK FROM HOME OPTIONS

DOING COACHING, COUNSELING AND ADVOCACY FOR ADULTS AND MINORS

Compensation:

• $40 inperson face visits in the home or in the community with clients plus mileage reimbursement at .55 mile. Visits must be a minimum of 30 minutes focused on client goals. Clients must verify visits by signing an electronic document. Compensation includes required documentation submitted timely.
• $25 for telehealth phone calls (minimum 15 minutes focused on client goals).

SUMMARY:

Work at home as well as in the community doing home visits while providing client services to our adult amd minor mental health clients. Assist clients with job development, coping and relationship skill building, arranging medical appointments, budgeting and applying for housing and entitlements, etc. Write progress notes and monthly summaries. Ensure all paperwork is accurate and up-to-date. Must be personable, engaging, have good verbal and written communication skills along with basic computer skills. Attends related training, staff meetings and supervision on a regular basis using Zoom.

PRINCIPAL RESPONSIBILITIES, DUTIES AND SKILLS

1. Use the computer to gather information and write notes. 2. Help clients develop and follow through on goals. 3. Provide hands on assistance helping clients apply for entitlements, housing, jobs, etc. 4. Makes home-visits and meet with clients in the community. 5. Writes brief notes after each interaction with client and keep Director abreast of services provided. 6. Collaborates with client's psychotherapist 7. Reports to the Clinical Director/Owner 8. Compensation includes doing the extras such as supervision, collaboration with client's psychotherapist and all documentation.
