Job Description
QMHP-CS Caseworker - Mental Health Services (Hybrid) Position Summary We are looking for a dedicated and compassionate Qualified Mental Health Professional - Community Services (QMHP-CS) to join our expanding behavioral health team. In this role, you will deliver individualized counseling, support, and case management services to a diverse population, including children, adolescents, adults, and individuals with developmental disabilities. Services are provided in a variety of settings, including clinic, community environments, and via telehealth. This opportunity is well-suited for professionals committed to supporting individuals with mental health needs while promoting stability, recovery, and long-term well-being. Key Responsibilities Complete comprehensive assessments to identify client needs Develop and carry out individualized, recovery-oriented treatment plans using evidence-based methods Provide individual and group skills training, along with supportive counseling Assist clients experiencing emotional distress, trauma, grief, behavioral challenges, substance use concerns, and crisis situations Apply crisis intervention and de-escalation techniques as needed Coordinate care through case management activities, including intake, service referrals, and ongoing support Connect clients with community resources and monitor progress Deliver services through telehealth when appropriate Maintain accurate, timely documentation in compliance with HIPAA and organizational standards Collaborate with a multidisciplinary team to support clients across the lifespan, including children, adolescents, individuals with Autism Spectrum Disorder, and older adults Address social determinants of health through advocacy and resource coordination Stay up to date on clinical best practices and documentation requirements Qualifications CANS and ANSA Certification required Experience in behavioral health, social services, or community mental health Experience working with children, families, or individuals with developmental disabilities preferred Knowledge of crisis intervention techniques Familiarity with evidence-based practices such as Motivational Interviewing and Cognitive Behavioral strategies Experience in outpatient or community-based settings preferred Telehealth experience is a plus Strong documentation skills and understanding of HIPAA compliance Ideal Candidate Compassionate and client-focused Collaborative and team-oriented Organized with strong attention to detail Committed to expanding access to mental health services Comfortable working with diverse populations Work Environment This hybrid role includes a mix of: Clinic-based services Community-based support Telehealth sessions Join a mission-driven organization focused on delivering accessible, high-quality mental health care and making a meaningful impact in the community.
