Job Description

Under the supervision of the Behavioral Health Program Manager, the Behavioral Health Counselor will deliver exceptional patient care. Evidence based services are provided both in-person and via telehealth in an integrated outpatient healthcare setting.
Duties & Responsibilities:
Provide psychotherapy services to individuals, families, and groups. Conduct ongoing assessments of patients' mental health, substance use, and psychosocial needs. Develop, implement, and regularly update individualized treatment plans that reflect patient goals, presenting symptoms, and progress. Evaluate the effectiveness of treatment plans and adjust interventions or level of care as clinically indicated, including short-term care, ongoing therapy, or discharge from the behavioral health program. Ensure proper documentation of notes, billing codes and more in the AHC electronic health record (EHR). Collaborate with other care teams including, but not limited to, Psychiatric Nurse Practitioners, Primary Care Providers, Medical Assisted Treatment (MAT) Providers, Case Managers and more to ensure comprehensive patient care. Collaborate with external partners to assess high-risk patients and refer for additional or alternative services wherever appropriate. Participate in AHC's performance monitoring processes. Work with data analysts to monitor key performance indicators, analyze trends, and establish goals to improve patient outcomes, and monitors quality measures and care gaps and implement strategies to improve outcomes. Maintains licensure and CEU requirements, regularly submitting records to the AHC Credentialing Team. Participate in continuous quality improvement projects as assigned. Collaborate with the revenue cycle team to ensure appropriate coding, documentation, and financial accountability. Other related duties as assigned.
Pay Range:
$50,000 - $65,000 annually based on qualification and experience.
